Disable as much windows stuff as possible. I run a bare-bones Windows XP with no Explorer.
Run MAME at realtime priority:
start /realtime mame
Use hardware scaling - set in mame.ini:
video ddraw hwstretch 1
Selecting secondary display:
-screen \\.\DISPLAY2
Switch resolution:
-switchres 1
Other important options:
-video ddraw -OR- -video d3d -keepaspect 1 -speed 1.0 -joystick 1
